• Location name and address : 5 Dayton Road, Waterford, CT 06385; 326 Washington St. Norwich, CT 06360; 584 Norwich Road, Suite 200, Plainfield, CT 06374Coverage Dates Needed : Variable depending on how long to credential, etc. Min 3 months effective immediately, beyond that is TBD. CT License required.Description of Work Assignment : Medical Oncologist- manage existing patient base for a provider on FMLA. His specialty is GI / GU but a generalist would certainly be considered. Benign hem also.APP- see follow-up oncology visits, perform chemo teaches, also a resource for emergent support for our onsite infusion centersProcedures or surgeries the provider will be required to perform : Bone Marrow Biopsies in-officeInpatient or Outpatient : outpatient but with the right provider could have inpatient component.Schedule or shifts needed : M-F; 8-4 : 30 but negotiable based on provider and availabilityOn call? Ideally for the MD, yes- every 3rd weekend and 1 day per week.-Confirming you will accept a physician or an APP- MD preferred, but could accept a skilled APP-Patient load expectations- full day ! depends on the patient type- follow ups may be 30 mins and new patients are 1 hour- assume 8 hours of patient facing time per day-Do you think you could do expedited creds to get someone in quickly? Ideally yes but I will need political support internally. It routinely takes 90 days minimum. So having a CT license helps and then I will lean on Kristi Gafford to help incentivize getting the team aligned on hospital / payor creds.
