A company is looking for an Analyst, Health Coach (Remote).

Key Responsibilities

Educate members on managing health concerns and empower group coaching participation

Assist members in collaborating with healthcare providers and care management teams

Document member interactions and follow up on personal action plans

Required Qualifications, Training, and Education

At least 2 years of experience as a Health Coach

Experience in care management, healthcare, social work, or aging networks

Associate's Degree required; Bachelor's preferred

Health Coach Certification preferred

Bilingual and community health experience preferred
