Job descriptionPrior experience as post-acute and long-term care Admissions Director, Admissions Coordinator or Admissions Screener. Ability to work independently and with Admission Directors and the interdiscplinary team Experience with electronic medical records including EPIC, Allscripts and Navi. Conducting sales and marketing duties at area hospitals and community events. The position is a hybrid of work in the community and in-house in our Dutchess County facilities. Meeting and maintianing relationships with patients and their families during the admissions process Previous experience with long term care is required. Hudson Wide Health Care has an exciting opportunity for an experienced Hospital Liasion / Marketer for SNF in Dutchess County area
Salary based on experience
DUTIES :
- Coordinating the referral and approval process in a timely manner.
- Establishing & maintaining relationships with local hospitals, physicians , assisted living , adult care centers & community referral sources.
- Complete the admission process from screeing to ensuring the arrival of the resident, to include all insurance and clinical documentation.
- Obtain insurance authorization for admission
- Independently conduce facility presentations and on-site tours.
