Patient Navigator - Oncology

Provides direct and indirect services for the navigation of Oncology patients and families according to assessed needs and the individualized patient plan of care authorized by the attending physician. Disseminates resources, educational, material, nutritional and financial, towards patient assistance that allows for access to oncology services. Assists and collaborates with organizational and outside resources for patient financial evaluation and assistance for treatment costs associated with oncology diagnosis.
