POSITION Summary

The Registered Nurse Case Manager position is responsible for providing comprehensive case management services in alignment with the American Case Management Association (ACMA) Standards of Practice, New Mexico Board of Nursing (NM BON) regulations, Det Norske Veritas (DNV) accreditation requirements and Critical Access Hospital (CAH) standards as regulated by the Centers for Medicare & Medicaid Services (CMS). This position collaborates with an interdisciplinary team to ensure appropriate utilization of services, compliance with regulatory requirements, safe transitions of care and optimal patient outcomes. This position serves as a clinical advocate, compliance steward and systems navigator, addresses the unique challenges of rural healthcare to improve outcomes, reduce avoidable hospitalizations and connect patients with appropriate community resources. MINIMUM MANDATORY qualifications

Experience :

• Minimum of two (2) years previous nursing experience.
• Associate’s Degree in Nursing from an accredited college or university.
