RN Case Manager - Compact License

A company is looking for a Transitions of Care RN Case Manager.

Key Responsibilities

Engage with newly discharged patients to assess their needs and develop care plans

Coordinate with healthcare providers and community resources for comprehensive patient care

Educate patients on chronic conditions and provide active care management to ensure adherence to treatment plans

Required Qualifications

3-4 years of RN Care Management or telephonic case management experience

An unrestricted Compact RN license obtained in the United States

Ability to work 8 : 00am - 5 : 00pm CST or MST

Experience in health insurance, particularly Medicare Advantage, is preferred

Prior experience in Special Needs Plan, Population Health, or Transitions of Care case management is desirable
