A Brief Overview
The position, Admission Coordinator, will assign patients to the appropriate team with the priority of geographic localization in a timely manner. The Admission Coordinator will serve as the conduit for improved patient outcomes and safety, enhanced communications between the team members, optimized access to orders and plans of care and safer Admission and discharges.
What You Will Do
- Manage the assignment and placement of patients the appropriate medical team. This includes patients from :
- Responsible, in collaboration with the Chief Residents and bed placement coordinator, for assigning patients to Department of Medicine clinical teams and consulting with the emergency room regarding appropriate placement for all in patients. Work collaborate with the CPFM team to assign patients to the appropriate level of care and if available geographic location.
- Assist the bed placement coordinator in assigning the patient to the appropriate patient care division to achieve appropriate level of care and if available optimal localization.
- Point person for all division secretaries and nurses to call regarding coverage questions.
- Aware of the admission caps for all services and assigns patients based upon the census of the teams and the number of Admission a given service may take. When all services are full, will problem solves with the Chief Resident and Program Director to find coverage for additional information.
- Work to resolve unclear coverage issues with the appropriate physician groups.
- Assist the Nursing Supervisor in the identification of actual and potential discharges.
- Advocate for appropriate patient placement based on admitting guideline (tele / ICU / observation) and will work with the Emergency Department and DACR to facilitate re-evaluation of admitting status for patients with long waits in the Emergency Department.

Decrease in the time for patient assignment from the ED
Increase in percentage of patients admitted to appropriate level of care and if available geographic location
Decrease in the number patients admitted to telemetry not meeting tele criteria
Decrease in LOS
Increase in compliance with CMS / IHI measures (Influenza, DVT, POA, etc.)
Increase in patient, nurse and resident satisfaction
Improvement in other designated patient outcome measures
Additional Responsibilities

(ADN) Associate's Degree in Nursing (Required)(BSN) Bachelor's Degree in Nursing (Preferred)Work Experience
5+ years RN experience, preferably as Advance Clinical Nurse (Required)ICU, ED, Nursing Supervisor or telemetry experience (Preferred)Experience within an Academic Medical Center (Preferred)Knowledge, Skills, & Abilities
