Job Summary
The Integrated Behavioral Health provider is responsible for providing comprehensive behavioral health services to individuals, families, and groups. This role focuses on assessment, diagnosis, treatment, and coordination of care for patients experiencing mental health and substance use disorders. As part of an integrated behavioral health and primary care practice, they provide initial patient evaluations, triage assessments, crisis intervention management, brief individual counseling, skills training, care coordination, case management, resource management, advocacy, and follow-up. They participate in interdisciplinary case conferences, work with primary care providers, and assist with program development.
Job Specific Duties and Responsibilities
Performs focused and timely behavioral health assessment, diagnosis, and treatment planning for patients, including referrals from other providers. Collects data from a variety of sources such as patient records, interviews, observation, and team members. Documents patient assessment and progress appropriately; maintains timely, complete and accurate records.Works as a member of the integrated primary care team. Contributes to team-based care through effective partnerships with patients, their caregivers, and other providers. Follows up on referrals in a timely manner with treatment recommendations. Communicates issues, variances and problems to other members of the care team in an appropriate and timely manner.Meets face-to-face with patient for immediate crisis intervention, behavioral health intervention or other social service-related issues. Participates and monitors the appropriate tracking system for identified patients. Coaches patients / families toward successful self-management.Serves as the point-of-contact, advocate, and informational resource for assigned patients, family, care team, payers and community resources. Proactively acts as patient advocate; responding with empathy and respect to resolve patient / family concerns.Builds and maintains effective working relationships. Serves as a liaison across disciplines. Participates as an active member of committees; leads work groups, projects and committees as assigned.Complete and maintain adequate and proper medical records with respect to all patients examined or treated in accordance with all applicable legal, accreditation and third-party payor standards within such time limits as are required by Hospital or its Medical Staff bylaws, rules, and regulations. Practitioner is subject to any fines and / or other penalties pertaining to delinquent medical records pursuant to Hospital's Medical Staff Bylaws.Performs special projects and other related duties as assigned.Maintain accurate and timely documentation in the electronic medical record (EMR) system.Participate in the resolution of patient and provider complaints and enhancement of patient and provider satisfaction as measured by regular surveys.Participate in community outreach activities at the request of District and as mutually agreed by the parties.Supervision of students and residentsPerform such other services as are mutually agreed by the parties.Essential Job FunctionOrganizational Responsibilities

Required Education and Experience
Master's degree or doctoral degree in Social Work or mental health counseling required from an accredited institution.Required Licenses, Certifications and / or Registrations
Unrestricted LMFT, LICSW, or LMHC license in the state of Washington.Current Basic Life Support (BLS) Certification.Required Knowledge, Skills, Abilities
Advanced knowledge of social work and behavioral health theories and practices. Demonstrated competence in clinical skills such that he / she is able to perform required tasks.Ability to assess patient needs and exercise independent judgment in managing and coordinating care.Ability to cultivate effective partnerships and work in collaboration with providers, other health care professionals, caregivers, and patients. Strong leadership, facilitation, delegation, and coaching skills.Proficient in the use of current technology, including Microsoft Office products. Ability to learn and effectively use electronic medical records and other systems and equipment.Professional and effective written and verbal communication skills. Ability to identify and employ communication strategies appropriate to the audience.
