Ascencia is a nonprofit homeless services agency based in Glendale, California, dedicated to helping individuals and families across L.A. County move from homelessness to permanent housing. Through street outreach, emergency shelter, case management, mental health care, and more, we provide compassionate, comprehensive support. With a 97% housing retention rate, our success reflects the dedication of our staff, volunteers, and community partners who make stability possible for those we serve.
This position provides high-quality, client-centered intensive supportive services to chronically homeless individuals, families, and transitional age youth in permanent supportive housing through Housing for Health (HFH). Services are grounded in harm reduction principles and aim to support participants in maintaining stable housing and improving overall well-being.
Duties and Responsibilities
Outreach, Engagement & Intake
- Accept and process referrals from DHS and HFH partners
- Build rapport with clients and assess eligibility for the program
- Obtain necessary authorizations and consents
- Refer ineligible individuals to appropriate services and community linkages
- Conduct client intakes and assist with gathering documentation
Temporary Housing, Rental & Move-In Support
Assist with temporary housing referrals prior to permanent housing placementSupport clients in lease-up processes and documentation coordinationCoordinate move-in logistics and new tenant orientationHelp clients access basic necessities such as food and hygiene itemsConduct DHS-approved psychosocial assessments and update service plans every three monthsEnsure connections to health, mental health, and substance use servicesSupport treatment adherence by accompanying clients to appointmentsProvide assistance with accessing public benefits and employment resourcesSupport budgeting, money management, and life skills developmentEducate clients on tenant rights and lease responsibilitiesWork with property managers to address issues affecting housing stability and document eviction prevention effortsCoordinate transitions for clients with changing needs or housing situationsImplement new services as required by County or HFH operationsClient Satisfaction
Monitor satisfaction through ongoing conversations and tenant meetingsAdminister an annual satisfaction surveyCollaboration
Participate in meetings with DHS, HFH, and community partnersFacilitate regular case conferencing with supervisor and teamBuild partnerships with local service providers to ensure wraparound supportQualifications and Requirements
Bachelor’s degree in Social Work or related field and 1 year of experience with homeless individuals, families, or transitional age youth; or 2 years of experience providing direct mental health or intensive case management servicesExperience in social services and working knowledge of case management techniquesExperience with clients facing mental health challenges, chronic health conditions, and substance use disordersUnderstanding of crisis intervention, harm reduction, and housing retention strategiesAbility to manage a caseload of 20 clients, with strong documentation and organizational skillsProficiency in Microsoft Word and ExcelValid driver’s license, ability to drive agency vehicles, and current auto insuranceCommitment to maintaining professional boundaries and client confidentiality per HIPAAAttendance at mandatory trainings and meetings requiredPhysical Working Conditions
Some travel required to field locations; valid driver’s license and insurance neededProlonged sitting, standing, and frequent bending or stretchingAbility to lift up to 25 lbs as neededFrequent computer use, including typing and phone communicationOther Conditions
