Job Overview :
Provide coordinator for all transplants from harvest coordination to communication with recipient and hospital staff. Provide patient management to patients being seen on an outpatient and inpatient basis. Work closely with attending physicians and office staff to ensure high-quality of patient care and patient satisfaction.

37.5 Hour Work weekFull TimeEssential Functions :
Assists in the completion disability, DME, prior authorizations or other forms when necessary in a timely manner.Attends all scheduled patient rounding and departmental meetings as assigned including but not limited to weekly selection committee or Quality Committee.Collect and collate pertinent data required for a thorough and complete evaluation according to departmental policies and guidelines in order to complete a patient work up and present the candidates to the “Selection” committee. Attend the weekly meetings. Documents discussion on all transplant decisions into transplant database.Communicates and reviews clinical findings to the nurse practitioners and physicians and follows up on any orders. Carries out physician orders or requests from nurse practitioners, and clarifies as necessary.Coordinates organ transplants by working with physicians, surgeons, patients, hospital departments, and OPO teams to ensure all logistics of organ the offer are coordinated and arranged most expeditiously. Ensure all necessary communication is done promptly.Obtain blood specimens and send to The New Jersey Organ & Tissue Sharing Network according to protocol and maintain a good working relationship with the lab and coordinator staff.Provide patient and family education regarding their disease process, transplant, pre- & post-operative care, medications, and discharge planning or any relevant information and document all education.Understands and complies with all policies and procedures regarding transplant or VAD patients. Ensure the the department is always compliant with TJC, CMS and UNOS guidelines.Benefits and Perks :
