Licensed Case Manager, DC / MD / VA

A company is looking for a Case Manager for a Hospital Readmission Reduction Program.

Key Responsibilities

Manage a caseload of members with mental illness and / or substance use disorders, ensuring successful discharge and community transition

Provide crisis intervention, coordinate care, and assist members with medication adherence and access to community resources

Document services provided and collaborate with treatment teams to evaluate and plan care for members

Required Qualifications

2-4 years of relevant experience in behavioral health case management or crisis intervention

Bachelor's Degree in psychology, social work, or a related field from an accredited institution

Knowledge of community resources and social determinants of health related to mental health and substance use disorders

Familiarity with benefits programs such as SSDI, SSI, Medicaid, and housing assistance

Proficiency in Google Suite and Microsoft Office software
