Regional Home Care Physical Therapist

JOB PURPOSE :

The Regional Physical Therapist oversees, plans, and administers physical therapy treatment programs for those medically referred patients to restore function, relieve pain, and prevent disability following disease, injury, or loss of body part.

This role involves the provision of physical therapy services in outpatient, clinic, and home settings across multiple locations within a geographic region.

In addition, this role will have a significant involvement in auditing and quality reviews. The regional Physical Therapist will be responsible for making recommendations to the Regional Rehabilitation Manager and Director of Rehabilitation regarding service delivery and documentation of services.

JOB RESPONSIBILITIES :

Direct Participant Care

• Perform participant evaluations and treatment at the PACE site, outpatient clinic, and / or in the field (participant's home, nursing homes, etc.)
• Documents all pertinent information as required;
• including but not limited to evaluations, progress notes, care plans, daily statistics, EMR etc.
• Participates in developing a patient care plan with interdisciplinary team members.
• Maintain a clean, safe environment and follow safety protocols.
• Maintains the inventory of supplies and equipment and recommends to the Director of Rehabilitation the purchase of necessary items for the proper functioning of the department.
• Provides clinical instruction, mentoring, and direction to PTA's, and Students as assigned.
• Only act within the scope of the individual's authority to practice.

QUALIFICATIONS : Education

Experience :

License : Current New York State Education Department Physical Therapy licensure / registration.

Additional Requirements :

Language : Bilingual preferred depending on region (participant demographics and location of PACE sites).

Must demonstrate agility and fine motor skills to operate and activate equipment, devices, instruments, and tools (ie. typing, use of medical supplies, equipment, etc.)
