Job Description :

Sunbelt Staffing is seeking a Home Health RN for a contract assignment in Riverhead, NY. This position requires experience with OASIS documentation and proficiency in Homecare Homebase (HCHB). The RN will deliver compassionate, skilled care to patients in their homes while maintaining accurate documentation and collaborating with the interdisciplinary care team.

Key Responsibilities :

Conduct patient assessments and complete admissions, resumptions, recerts, and discharges using OASIS

Provide direct patient care and education in the home setting

Document care accurately in Homecare Homebase (HCHB)

Communicate and collaborate with physicians and healthcare professionals

Manage a caseload efficiently while maintaining high-quality standards

Qualifications :

Active RN license in New York

Minimum 2 years of nursing experience, including at least 1 year in home health

OASIS and Homecare Homebase (HCHB) proficiency required

Reliable vehicle, drivers license, and auto insurance

Ability to pass background check, drug screen, and onboarding in compliance with the Joint Commission
