Job Summary / Objective :

The Behavioral Health Therapist provides counseling services to participants in the behavioral health program, offering individual therapy sessions. Acting as a liaison, the therapist facilitates communication and care coordination between patients, medical providers, family members, and other relevant contacts. Key responsibilities include assessing and addressing behavioral issues related to medical conditions, maintaining accurate treatment records, and ensuring compliance with privacy and safety protocols.

Sign-On Bonus :

Essential Functions / Duties / Responsibilities :

• Provide individual therapeutic counseling to address patients' behavioral health concerns.
• Collaborate with medical providers, patients' families, and other relevant contacts to support the development and implementation of comprehensive care plans.
• Assess patients for behavioral issues related to medical conditions or treatments and develop effective strategies for resolution.
• Create and implement individualized treatment plans tailored to patients' needs and goals.
• Monitor patient progress and adjust treatment plans as needed to achieve optimal outcomes.
• Accurately document all patient interactions, assessments, and treatment plans in accordance with privacy and safety regulations.
• Educate patients and their families on managing mental health and behavioral challenges.
• Refer patients to appropriate community or hospital resources to ensure holistic care.
• Participate in interdisciplinary care team meetings to discuss patient progress and refine care strategies.
• Stay current on best practices, ethical standards, and developments in the field of behavioral health therapy.

Education, Certificates, and License : Education : Masters degree in social work, Counseling, Psychology, or a related field from an accredited institution required.

Certificates / License : Current licensure or board certification as a Licensed Clinical Social Worker (LCSW), Licensed Master Social Worker (LMSW) or Mental Health Counselor (MHC). Additional certifications in behavioral health or specific therapeutic modalities preferred.
