Position Summary

The Home Care / Hospice RN provides patient-centered, compassionate care to patients in their homes or facility settings, focusing on comfort, symptom management, and quality of life. This RN will independently manage a caseload, perform routine and PRN visits, and collaborate closely with the hospice interdisciplinary team. The role requires strong clinical judgment, excellent time-management skills, and proficiency using HCHB for documentation.

The role requires strong clinical judgment, excellent time-management skills, and proficiency using HCHB for documentation.\n\nKey Responsibilities\nCase Management & Patient Visits\n\nManage an assigned caseload of hospice or home care patients.\n\nConduct comprehensive nursing assessments in homes or facilities.\n\nDevelop, implement, and revise individualized plans of care.\n\nProvide pain and symptom management consistent with hospice standards.\n\nPerform scheduled visits, PRN visits, and crisis visits as needed.\n\nDocument all care accurately and timely in HomeCare HomeBase (HCHB).\n\nPatient & Family Support\n\nEducate patients and families on disease progression, hospice philosophy, medication management, and end-of-life expectations.\n\nOffer emotional support and help families navigate critical decisions.\n\nEnsure patient comfort, dignity, and safety at all times.\n\nInterdisciplinary Coordination\n\nCollaborate with physicians, social workers, chaplains, CNAs, and other hospice team members.\n\nParticipate in IDT (interdisciplinary team) meetings and communicate patient changes promptly.\n\nCoordinate services such as DME, medications, and ancillary support.\n\nCompliance & Quality Standards\n\nFollow all hospice regulations, Medicare requirements, and agency policies.\n\nMaintain accurate documentation, care plans, and visit notes.\n\nUphold infection control standards and safety procedures during field visits.\n\nField-Based Responsibilities\n\nTravel throughout the service area daily; must be able to work independently.\n\nManage time effectively to meet visit requirements and patient needs.\n\nMaintain professional conduct, confidentiality, and strong customer service skills.\n\nQualifications\nRequired : \n\nActive RN license (state-specific; must be active by start date).\n\nHomeCare HomeBase (HCHB) experience.\n\nPrior travel RN experience.\n\nMinimum 1 year hospice or home health case management experience.\n\nValid driver's license, reliable personal vehicle, and auto insurance.\n\nStrong communication, assessment, and critical-thinking ability.
