Job Requirements

Education Requirements : Master's Degree in Physical Therapy from an accredited institution
Certifications Requirements : AHA BLS certification, valid State of Wisconsin Licensure, NPI, Drivers License with proof of insurance, any required professional certifications
Experience Requirements : Minimum of 1 year of Home Health experience working with adult and geriatric populations, orthopedic, neurological, and functional mobility cases
Other Requirements : Ability to work independently, strong communication skills, resilience, adaptability to change, and social / interpersonal skills

Job Responsibilities

Provide comprehensive physical therapy evaluations, treatment planning, and interventions for adult and geriatric patients in a home health setting.

Assess patient mobility, neurological status, and orthopedic needs to develop individualized care strategies.

Document patient progress and treatment outcomes accurately using NetSmart EMR system, meeting all regulatory requirements.

Educate patients and caregivers on therapeutic exercises, mobility techniques, and safety measures to enhance recovery and independence.

Collaborate with supervising physicians and interdisciplinary team members to optimize patient care plans and improve health outcomes.
