Position Summary

The Community Health Worker (CHW) – Outreach Specialist will play a critical role in connecting members with Wider Circle’s programs through remote outreach operations. This experienced CHW will leverage their expertise to conduct inbound and outbound calls, foster member relationships, and promote participation in community health initiatives. The role requires a combination of exceptional communication skills, a proactive mindset, and a commitment to member engagement and satisfaction.

Key Responsibilities

• Conduct outbound calls to engage members, promote programs, and build trust.
• Manage inbound calls, addressing member inquiries with professionalism and empathy.
• Provide program information, resolve member concerns, and support their participation in health-focused initiatives.
• Execute targeted outreach campaigns, ensuring consistency and accuracy in messaging.
• Maintain detailed and accurate records of member interactions in the outreach center database.
• Collaborate with the team and leadership to refine outreach strategies, share best practices, and meet organizational goals.
• Consistently achieve or exceed call targets and performance metrics.
• Stay informed about company programs, updates, and outreach techniques through ongoing training.
• Demonstrate cultural competence and understanding of the communities being served.

Requirements

Skills :

Technical Requirements : Reliable, fast-speed internet connection, a quiet and professional remote work environment, and basic computer literacy and accessing, downloading, and using specialized software.

Preferred Qualifications

Familiarity with health and social services systems and a deep understanding of community health principles.

Experience working with diverse populations and addressing social determinants of health.

Physical Demands :

Prolonged Sitting : This role requires sitting for extended periods while working at a desk or workstation.

Repetitive Tasks : Frequent and repetitive activities such as making outbound calls, answering inbound calls, and typing notes or data into the system.

Manual Dexterity : Regular use of a computer keyboard, mouse, and telephone is required, demanding hand-eye coordination and fine motor skills.

Visual Focus : Sustained use of computer screens and reading written or digital materials.

Communication : Extended periods of speaking and listening, often requiring clear enunciation and focused attention.
