REGISTERED NURSE - HOME HEALTH CASE MANAGER

LOCATION : WESTERN SUBURBS WITH FOCUS ON -Maywood, Bellwood, Riverside, Broadview, Berkely, Elmhurst, Northlake, Stone Park, Hillside, Forest Park, Cicero, Berwyn, Stickney

HOURS : DAYS WITH ROTATION TO WKDS AND HOLIDAY S

This position is also eligible for up to a $10,000.00 sign on bonus and subject to all taxes! You must be employed in the exact position at time of payout.  Internal team members are not eligible for this sign on bonus at this time.

Major Responsibilities :

Demonstrates adaptive communication styles based on individual patient needs

Mobilizes appropriate resources in response to situations that have the potential to negatively impact patient and family outcomes

Assesses patient and family readiness to learn, and modifies approach as necessary

Recognizes changes in clinical situations. Evaluates and implements changes to the plan of care

Participates in unit meetings and activities that improve patient care or administrative systems Utilizes available resources to maintain current knowledge base relevant to practice

COMPENSATION : $78,000.00-$117,000.00 annual base salary. This position is incentive eligible.
