POSITION SUMMARY
We are pleased to offer the opportunity to work as a Case Manager within the DuPage County Department of Community Services. Case Managers provide comprehensive case management and / or service coordination in a variety of areas. The current Case Manager opening is in the following programs / assignment area :
Long Term Care Ombudsman- Case managers in this programrepresent and advocate for residents, age 60 and older, in long term care facilities, licensed assisted living and supportive living facilities. They receive, investigate, and attempt to resolve concerns and complaints regarding the care of residents as well as providing a regular presence in the licensed facilities.
This is a grant funded position.
