The Hamlet Rehabilitation and Healthcare Center at Nesconset is a compassionate and resident-centered Skilled Nursing Facility dedicated to providing high-quality care and support. We are seeking a dedicated and empathetic Social Worker to join our interdisciplinary team and make a meaningful impact in the lives of our residents and their families.
Social Worker Position Summary :
The Social Worker is responsible for providing emotional support, discharge planning, and case management services to residents and families. This role works closely with Nursing, Admissions, Therapy, and Administration to ensure smooth transitions, resident satisfaction, and regulatory compliance.
Social Worker Key Responsibilities :
Conduct psychosocial assessments and care planning upon admission and throughout the resident’s stay.
Provide emotional support, counseling, and crisis intervention to residents and families.
Coordinate discharge planning, including placement, home care services, and community resources.
Serve as a liaison between residents, families, staff, and external agencies.
Participate in interdisciplinary care plan meetings and maintain accurate documentation.
Advocate for residents’ rights and support their physical, emotional, and social well-being.
Address concerns, grievances, and family communication in a professional and timely manner.
Ensure compliance with federal, state, and facility regulations related to social services.
Social Worker Qualifications :
Bachelor’s Degree in Social Work (BSW) required; MSW preferred.
Previous experience in a Skilled Nursing Facility or healthcare setting strongly preferred.
Strong communication, problem-solving, and organizational skills.
Knowledge of discharge planning, psychosocial needs, and LTC regulations.
Compassionate, patient, and resident-focused approach.
