Physical Therapist (PT)

We are seeking a skilled and motivated Physical Therapist to provide high-quality rehabilitative care aimed at improving mobility, strength, and functional independence. The PT will evaluate patient needs, create individualized treatment plans, deliver therapeutic interventions, and support patients throughout their recovery journey.

Responsibilities:

• Perform evaluations to assess mobility limitations, pain levels, and functional impairments.
• Develop personalized treatment plans focused on improving movement, balance, and physical function.
• Provide therapeutic exercises, manual therapy, and other evidence-based interventions.
• Document assessments, treatments, and progress clearly and accurately.
• Communicate and collaborate with healthcare team members to coordinate patient care.
• Educate patients and families on home programs, safety techniques, and rehabilitation goals.

Qualifications:

• Current state licensure as a Physical Therapist.
• Experience in outpatient, inpatient, rehab, or long-term care settings preferred.
• Strong communication, problem-solving, and clinical assessment skills.
• Ability to work effectively with diverse patient populations and interdisciplinary teams.
