Barton is seeking a skilled Functional Medicine Nurse Practitioner for a telehealth assignment. The position starts immediately, with a schedule of 3 days per week, and potential for full time extension. Working hours are typically 8 : 00 AM to 5 : 00 PM. The NP will focus on hormone replacement therapy, thyroid optimization, weight loss, metabolic support, and various wellness-focused consultations. • Conduct initial consultations and new patient lab reviews via video\n\n
• Review and interpret advanced labs (GI-MAP, Methyl Detox, ALCAT, hormone panels)\n\n
• Develop personalized care plans including lifestyle, nutrition, and supplementation\n\n
• Manage asynchronous follow-up care via EMR platform\n\n
• Educate patients on test results and coach on health optimization\n\n
• Complete all documentation in EMR (CHARM / CANVAS) within 24 hours\n\n
• Provide concise, functional medicine-informed SOAP notes\n\n
• Communicate with support staff to coordinate care\n\n
• Prescribe and manage bioidentical hormones, peptides, and nutraceuticals\n\n
• Monitor patient response and adjust dosing as needed\n\n
• Collaborate with compounding pharmacies and support teams\n\nAdditional Information : \n\n
