RN Case Manager

A company is looking for a Case Manager RN.

Key Responsibilities

Assess member health status and care coordination needs, including inpatient review and discharge planning

Coordinate with patients, families, physicians, and healthcare providers to develop and monitor treatment plans

Identify and implement cost-effective treatment alternatives while ensuring quality care and compliance with healthcare guidelines

Required Qualifications

5+ years of related work experience in clinical nursing and patient assessment

Bachelor's degree or equivalent related work experience

Graduate of an accredited school of nursing

RN - Registered Nurse Licensure with Compact State Licensure in good standing

Knowledge of medical terminology and cost containment practices in healthcare
