- Active Vermont RN license or compact state eligibility\n

- Current BLS certification (American Heart Association)\n
- Proficient in OASIS documentation and electronic medical records\n
- Strong communication, organizational, and time management skills\n
- Ability to work independently and manage a dynamic homecare caseload\n
- Valid driver's license and reliable transportation required\n\nHomecare RN Responsibilities\n
- Conduct comprehensive patient assessments and create individualized care plans for home health patients\n
- Serve as case manager, efficiently managing caseloads and visit schedules\n
- Implement medical plans of care, nursing interventions, and procedures for safe, high-quality patient care\n
- Complete and maintain accurate OASIS and agency documentation per state and federal regulations\n
- Educate patients and caregivers on medications, treatments, and homecare processes\n
- Coordinate interdisciplinary care with physicians, therapists, social workers, and healthcare team members\n
- Maintain infection control standards and demonstrate proficiency in high-tech nursing skills (IV therapy, wound care, wound VAC)\n
- Participate in peer review audits, case conferences, and quality improvement initiatives\n
- Ensure timely and safe patient discharges or transfers, completing all required documentation\n
