Job description

LanceSoft is seeking a travel Physical Therapist for a travel job in Glendale, California.

Job Description & Requirements

Position Details :

  • Field-based Home Health PT role servicing Baldwin Park and surrounding areas
  • Home Health experience preferred , but willing to consider candidates without prior HH experience
  • CA Physical Therapy License preferred – candidates with pending CA license must have applied prior to submission
  • OASIS experience is required (Outcome and Assessment Information Set)
  • Guaranteed hours based on completed visits :

    • 25 completed visits = 40 hours per week

  • Hours guaranteed if 25 or more visits completed weekly
  • COVID exemptions permitted
  • Mileage reimbursement at IRS standard rates between patient visits
