The County of Santa Barbara Health is accepting applications for a Reentry Coordinator (Licensed) to oversee implementation and coordination of reentry services, including CalAIM Justice-Involved Jail Reentry Bundle services . The County classification title is Health Services Manager II . This position is "At-Will" and exempt from Civil Service protections.

Through collaboration amongst the partner agencies and the leadership of the Reentry Coordinator , Santa Barbara County will ensure individuals entering the criminal legal system receive multi-agency assessments and case planning to assist in their successful return to the community at the earliest stage possible based on their risk and needs, and the statutorily available options. This approach will assist in ensuring all diversionary options are maximized and individuals exiting the criminal legal system (pretrial and post sentence) have a robust discharge plan. This work will interface with work performed by multiple agencies between the Jail Discharge Planning Team, County Health Department, the Sheriff's Office, Probation, County Executive Office, Public Defender's Office, Managed Care Plans, California Department of Corrections and Rehabilitation, and a range of community-based service providers.

Under direction, the Reentry Coordinator will lead the operational oversight and coordination of services for individuals reentering the community from the County's two jails, with a primary focus on implementing CalAIM justice-involved initiatives, including Jail Reentry services. This role will manage the integration of service delivery between the jails, and Enhanced Care Management (ECM) and Community Supports, working closely with the Medi-Cal managed care plan to ensure seamless transitions of care. They will provide clinical oversight to staff delivering reentry services including maintaining compliance with state licensure standards and clinical best practices. This includes supervision, training, and evaluation of clinical and care coordination staff, as needed, and will participate in regional and statewide planning efforts related to reentry services.

They will develop and implement protocols and workflows that support continuity of care for justice-involved individuals, ensuring these systems are responsive, equitable, and efficient. The position is also responsible for monitoring program data, ensuring accurate and timely reporting that aligns with CalAIM and Medi-Cal managed care plan requirements.

This position plays a critical leadership role in bridging behavioral health, public health, and correctional systems to support individuals during their transition from jail to community-based services, including coordinating care for Medi-Cal eligible clients. The ideal candidate will be a Licensed Behavioral Health Professional OR Nurse with strong program management and interagency coordination skills.

The IDEAL CANDIDATE will possess :

  • Extensive experience working with justice-involved individuals or high-needs populations in correctional or community settings, including reentry and / or behavioral health services.
  • Knowledge of CalAIM, Enhanced Care Management (ECM), and Community Supports.
  • Strong leadership and supervisory skills in a clinical or health program setting.
  • Ability to collaborate across agencies and manage complex care transitions.
  • Deep understanding of the Medi-Cal system, behavioral health, social services, and public health systems.
  • Commitment to health equity, culturally competent care, and trauma-informed practice.
  • Examples of Duties

  • Serves as a member of the County's management team by planning, organizing, coordinating, and directing the activities in areas such as : public health, mental health and recovery, correctional health, and / or medical health programs.
  • Establishes annual goals and priorities, influences the direction for new or revised services, programs, processes, standards, or operational plans, based upon the County's longer-term strategies.
  • Directly supervises the Jail Reentry Care Managers who have responsibility for creating reentry plans with individuals housed in jail.
  • Plans, coordinates, and evaluates health services programs and / or projects in areas including, but not limited to public health, mental health and recovery, correctional health, and medical health programs and projects.
  • Serves as a subject matter expert and uses technical and analytical expertise to develop findings.
  • Directs, coordinates, and participates in the development and implementation of program goals, objectives, policies, procedures, and priorities that balance both mandated services and community needs.
  • Negotiates, develops, and manages contracts; and monitors and evaluates for compliance with contractual obligations.
  • Researches, analyzes, develops, and reviews studies, allocations, and related reports, contracts, proposals, materials, presentations, and other documents; and develops and makes recommendations to higher-level staff.
  • Develops, implements, and evaluates services and programs to ensure adherence to strategic and performance goals; manages work systems and processes; maintains and updates procedures; researches trends and best practices in area of assignment; and recommends and implements operational and / or program changes.
  • Develops and administers budgetary and procurement processes; manages contracts; researches and analyzes financial information; makes resource allocation recommendations; and implements resource allocation decisions.
  • Troubleshoots and resolves program or work unit issues and keeps higher-level management informed of issues / resolution.
  • Oversees the preparation of or prepares contracts, reports, records, plans, and other documents involved in managing the area of assignment; oversees and participates in the research and analysis of policy and related information; and makes recommendations to higher-level management staff.
  • Supervises management, professional, paraprofessional, and support staff; and is responsible for conducting performance evaluations, employee development, hiring, discipline, and termination procedures.
  • Coordinates work with the general public, County staff, and external agencies; may represent the County on various professional committees; and may prepare and make presentations to the public, elected officials, boards, and other stakeholders.
  • Oversees contracted providers in diagnostic and / or clinical settings.

  • Employment Standards

    The knowledge, skills, and abilities listed below may be acquired through various types of training, education, and experience. A typical way to acquire the required knowledge and abilities would be :

  • Equivalent of a bachelor's degree from an accredited four-year college or university in Social Work, Psychology, Nursing, Public Health, Health Administration, or a closely related field AND ,
  • Five (5) years of progressively responsible experience in public health, behavioral health, or healthcare program, including at least two (2) years as a supervisor or manager.
  • Licenses and Certificates

    Must possess and maintain a current, valid license in the State of California as one of the following :

  • Licensed Clinical Social Worker (LCSW)
  • Licensed Marriage and Family Therapist (LMFT)
  • Licensed Professional Clinical Counselor (LPCC)
  • Licensed Clinical Psychologist (LCP)
  • Doctorate in Psychology (PhD or PsyD)
  • Registered Nurse (RN)
  • For specific positions, possession and maintenance of a valid California driver's license and the ability to meet automobile insurability requirements of the County or the ability to provide suitable transportation needs as a condition of continued employment.

    In addition to the Live Scan, the following process will be used as part of the background investigation process after the candidate is extended a conditional offer of employment for the County Health Department :

  • Personal History Questionnaire (PHQ). The selected individual will receive a notice regarding the completion of a Personal History Questionnaire (PHQ). Failure to submit a PHQ by the deadline will result in disqualification from the hiring process for the County Health Department.
  • Background Investigation : The selected individual will be required to complete a Personal History Statement (PHS) and undergo an extensive background investigation that includes an evaluation of references, prior employment, drug history, residence history, and neighborhood check. This process may take 4-6 weeks.
  • Current or excessive use of drugs, sale of drugs, serious criminal history, or deception relating to employment history may bar employment.

    The County of Santa Barbara Health Department (County Health) requires all employees working in direct contact with patients or clients in a high-risk environment to comply with the County Health Employee Immunizations policy. This can be accomplished by providing documentation of immunizations, submitting laboratory immune titers demonstrating adequate immunity, or obtaining required vaccinations. A list of requirements, based on the specific program, will be sent at the time an offer letter is presented to the selected candidate(s) and initial documentation will be required at this time.

    Some positions in this class may require as a condition of employment that the incumbent sign a statement agreeing to comply with Section 11166 of the California Penal Code relating to child abuse reporting and Section 15630 of the Welfare and Institutions Code relating to dependent adult abuse reporting.
