This is a grant funded position.Position Overview: The Registered Dietitian is responsible for assessing patients' nutritional needs, developing personalized dietary plans, and providing education on healthy eating habits. The Dietitian will work together with a multidisciplinary healthcare team to incorporate nutritional interventions into the comprehensive patient care plan. The position responsibilities also include building and maintaining relationships with community partners.
Duties & Responsibilities: - Provide nutrition counseling and education to individuals and groups, addressing specific health conditions and dietary concerns. Including but not limited to diabetes, heart diseases, hypertension, obesity, malnutrition, and kidney diseases.
- Monitor patients' progress and adjust nutrition plans as needed.
- Document patient interactions and maintain accurate records.
- Collaborate with healthcare professionals to integrate nutrition interventions into overall patient care and providing referrals to healthcare professionals as appropriate.
- Participate in community outreach programs to promote nutrition awareness and preventive healthcare.
- Provides monthly, quarterly and annual program reports.
- Stay updated on the latest nutritional research, guidelines, and trends to enhance patient care and other duties as assigned.

RequirementsEducation: - Bachelor's degree in nutrition, Dietetics, or related field.
Experience: - At least one (1) year of experience in a clinical health setting.
Licenses/Certifications: - Registered Dietitian credentials and state license.
- Valid TX Driver's License.
Knowledge, Skills and Abilities: - Proficient in MS Office Suite (Word, Excel, PowerPoint, Outlook).

General InformationPosition Type and Typical Hours of Work: - This is a full-time | grant-funded position.
- Days and hours of work are typically Monday through Friday from 8:00 am to 5:00 pm.
- Expected to travel within Harris County jurisdiction.
Work Environment: - This job operates in both a professional office and community environment.
- This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets, and fax machines.
Physical Demands: - This role requires the ability to lift, carry, push, pull up to 50 pounds and bending & standing for periods at a time.
