Men's Case Manager

Qualifications :

• Associates or Bachelor's degree in Human Services, Psychology, Counseling or related field and / or two years' experience in working with behavioral health consumers
• Experienced in providing trauma informed care
• Ability to work in cooperation with internal and external personnel and agencies
• Ability to work with and advocate for individuals with diverse backgrounds
• Is person-centered in approach and possesses strong critical thinking skills
• Possesses strong crisis prevention and intervention skills
• Certified in First Aid, CPR, and as a Medication Aide before completion of probationary period
• Has a valid Nebraska Driver's License and is insurable with a good driving record
• Strong organizational, interpersonal, written and verbal communication skills
• Ability to perform efficiently and accurately in a fast-paced, deadline-oriented work environment.
• Ability to multi-task when necessary
• Possess exemplary stress management skills
• Possess a non-judgmental, caring attitude toward persons served
• Strong computer skills. Experience with electronic health records preferred
• Familiarity with CARF standards and a willingness to conform to all standards
• Ability to work full-time (a minimum of 40 hours a week) to include evenings and / or weekends as required
• Ability to work successfully as a team member, as well as independently

Duties and Responsibilities :

Complete the initial needs assessment with new admissions.

Complete initial respite paperwork to include the Needs Assessment with new respite admission

Meet with each resident weekly to assist in completion of case management goals.

Track and report case management activities to the treatment team.

Oversee and manage the Level System.

Begin discharge planning from the time that the consumer admits. Proactively work toward a strong discharge plan.

Probationary Period : Six months
