Shift : 8-hour days, Every Other Weekend

Guaranteed Hours : 40 hours per week

Experience : Minimum of 2 years

License : CA RN License

Certifications : BLS

Must-Have : Prior experience in Care Coordination

Job Description :

• Manage and coordinate patient care to ensure effective use of resources and high-quality healthcare services.
• Collaborate with interdisciplinary teams to evaluate patient needs and develop care plans.
• Utilize the Cerner charting system for accurate and efficient documentation.
• Participate in case reviews and contribute to the development of strategies for optimal patient outcomes.
• Uphold hospital and unit policies, with flexibility to adapt to changing patient and unit needs.
• Participate in a mandatory 12-hour orientation program prior to shift commencement.
• Comply with a cancellation policy that allows cancellation of up to 4 shifts during the assignment.
• Maintain clear and professional communication with healthcare teams and patients.
• Ensure adherence to federal and state regulations in healthcare provision.
