Schedule :  Full-Time / Day Shift. 8am-5pm (weekend coverage rare)

Location :   Covering Admissions in Lagrange, Hinsdale, Bolingbrook and Glen Oaks Hospitals and some community admissions.

The role you’ll contribute :

The Admissions Nurse is responsible for responding to and visiting with patients who have been referred for hospice consultation and or admission to AdventHealth Hospice. The Admissions Nurse assesses the appropriateness / qualifications of the patient for Hospice services and informs the patients, caregivers, and family members of the hospice services available with the organization. In addition, the Admissions Nurse is responsible for visiting current AdventHealth Hospice patients who have been admitted to an AdventHealth hospital.
