This position involves providing management and delivery of patient care plans across Western PA and surrounding areas. The role includes weekend on-call requirements and local travel to patient locations. Candidates should have an associate degree or higher an active RN license in Pennsylvania and at least one year of RN experience.

Key Responsibilities

Lead and collaborate to provide management and delivery of patient care plans

Support patients throughout Western PA and surrounding areas

Drive approximately 20% of the workday for patient visits

Meet critical deadlines for charting and documentation

Self-manage time and schedule to meet patient needs

Schedule and Shift Details

On-call Friday Saturday and Sunday from 5 : 00pm to 10 : 00pm

8 units required on Saturday

8 units required on Sunday

Two holidays per year

Travel

Local travel required

Qualifications

Associate degree or higher

At least one year of experience in any of the following : clinical documentation clinical informatics clinical quality

At least one year of experience with electronic medical record systems including EMR Other EMR or Practice Partner EMR

Valid Registered Nurse (RN) license in Pennsylvania

Valid certifications including : American Red Cross Advanced Life Support (ARC ALS) and American Heart Association Advanced Cardiac Life Support (AHA ACLS)

Willingness to meet travel requirements
