Dates : ASAP - Ongoing

Practice Setting : Outpatient, primary care clinic with rural health clinic designation

Schedule : Monday-Friday, 8 : 00 AM - 5 : 00 PM

Call Schedule : N / A

Number of Physicians in Practice : 4 (+2 per diem)

Number of APPs : 3

Average Number of Patients per Day : Outpatient 18
