Job descriptionAssess emotional factors related to terminal illness, as well as environmental resources and obstacles to maintain safety Provide emotional support, short-term crisis intervention, and individual and / or family counseling Maintain contact with appropriate community agencies and services in order to promote interagency cooperation and to facilitate related referrals Evaluate patient / family response to psychosocial interventions Assess special needs related to cultural diversity, including communication, space, role of family members, and special traditions Perform comprehensive initial psychosocial assessments Assess the patient / family psychosocial status, potential for risk of suicide, and / or abuse or neglect Perform other duties, as needed Master's Degree in Social Work Active Massachusetts Licensed Clinical Social Worker (LCSW) and / or Licensed Independent Clinical Social Worker (LICSW) Solid understanding of Hospice philosophy related to families in crisis and end-of-life issues Strong verbal and written communication skills 1+ year of related Social Work experience Hospice background
