The Licensed Practical (LPN) is an integral part of the care team that helps support the lives of our patients and residents each day.

Responsibilities Include :

• Provide bedside nursing
• Care planning modifications and implementation
• Managing Patient Care provided by CNAs
• Communicate with IDT members and providers
• Medication administration
• Provide treatments
• Provide information and compassion to residents and family members
• Completing incident reports as needed and participating in investigations
• Documenting accuratley in the resident record
• Maintaining organizaiton of the nursing station and medication / treatment carts

Required qualifications :

Graduate of approved Licensed Practical Nurse (LPN) Program.

Previous experience as an LPN in an acute care / long-term care setting is desirable.

Current licensure to practice as a Licensed Practical Nurse in the State of New York.

Job Types : Full-time, Part-time or Per Diem available

Day, Evening and Night shifts available
