SCOPE OF ROLE

Reporting to the Supervisor of Behavioral Health Services, the Case Manager, In-Home Youth / Family (QMHS-CBFT) delivers mental health services to children, adolescents, and adults in home, school, and community settings. The Case Manager, In-Home Youth / Family supports clients with emotional or behavioral challenges by promoting stability, independence, and improved functioning. The role supports a small, dedicated, shared case load of 10-11 youth / families in Lake County with the Counselor and Case Manager. Team consultations occur multiple times per week with support from Supervisor. No on-call requirements are necessary for this role, and it offers a regular Monday - Friday, day shift schedule.

The Case Manager, In-Home Youth / Family connects individuals and families to community resources, including transportation as needed, and advocates for client needs. Using a client-centered, strengths-based approach, this position engages clients in treatment, coordinates care across systems and supports those at risk for out-of-home placement or psychiatric hospitalization. The Case Manager, In-Home Youth / Family builds strong, collaborative relationships with clients, families, and Signature Health team members.
