Are you passionate about patient care and creating a positive first experience for those navigating chronic health conditions? We’re looking for Care Coordinators (CTC) to join our growing team and play a key role in supporting the onboarding of patients into our Chronic Care Management (CCM) program.
About the Role
As a CTC, you’ll be responsible for educating patients on CCM services, obtaining consent, creating the first care plan, and ensuring a smooth relationship with your patients as you help then navigate through CCM. This is an excellent opportunity for someone who thrives in a collaborative, patient-centered environment and enjoys a mix of communication and administrative responsibilities.
What You’ll Do
- Conduct initial outreach calls to introduce the CCM program and obtain patient consent.
- Develop and document the first care plan in alignment with CCM requirements.
- Complete the initial CCM report.
- Ensure all documentation is accurate, timely, and compliant with regulatory standards.
- Develop medical relationships with Patients
- Address any questions or concerns patients may have during the onboarding process.
- Support various administrative tasks related to enrollment and care coordination.

Compensation features supplemental pay : earn per patient served with a guaranteed minimum hourly wage of $16–$17 , based on licensure and experience .
