Registered Nurse – Home Health

Location : Confidential – Virginia

Schedule : Monday–Friday, 8 : 00 AM – 5 : 00 PM

Hours : 40 hours per week | 8 hours per day

Duration : Contract assignment

Overview

We are seeking experienced Registered Nurses (RNs) to provide compassionate home health care. This role focuses on supporting patients in maintaining independence and dignity while following established plans of care. Candidates should be comfortable traveling to patient homes, providing skilled clinical services, and working collaboratively with interdisciplinary teams.

Responsibilities

Deliver care according to individualized plans of care, ensuring patient dignity and independence.

Monitor vital signs and promptly report abnormal findings.

Administer medications, IV therapy, wound care, feedings, and oxygen therapy as prescribed.

Assist with ostomy care and other clinical interventions as needed.

Collaborate with physical, occupational, and speech therapists to support continuity of care.

Follow infection control policies and report potential communicable diseases.

Document all nursing actions, progress notes, and outcomes accurately and timely.

Order and maintain adequate supplies, medications, and equipment.

Monitor and report patient incidents; ensure follow-up and incident documentation is completed.

Ensure proper use and maintenance of medical equipment, reporting any malfunctions as needed.

Qualifications

Active RN license (state requirement).

Preferred experience : Home Health, HCHB (HomeCare HomeBase) system, wound care, and IV therapy.

Strong patient communication and bedside manner.

Ability to work independently while collaborating with an interdisciplinary team.

Flexibility to take on-call or overtime assignments, if needed.
