As the Clinical Care Provider for the Hope Hospice Center, Partners In Home Care is dedicated to delivering compassionate, expert care that upholds the dignity, comfort, and unique needs of every patient and their loved ones.
Our PRN staff help cover shifts based on patient / staffing needs.
- Must be available for both day and night shifts. Shifts are 6am-6pm / 6pm-6am.

In collaboration with the Hope Foundation Partners In Home Care will be providing clinical operations at the new Hope Hospice Center.
Your role as a CNA is not just a job but a heartfelt opportunity to provide comfort and support to those nearing the end of life. Our CNAs provide personal healthcare and related services to patients in the Hope Hospice Center. This position is vital in creating a safe and clean environment, working cooperatively with patients and their families. Our CNAs are key members of each patients care team, committed to making a meaningful difference daily.
Core Responsibilities
Patient-Centered Care : Provide personal care in various settings the patient prefers, including homes, nursing homes, assisted living facilities, eldercare facilities or community partners inpatient care settings.Observation and Reporting : Act as the eyes and ears for the broader hospice team, observing the patients condition and reporting any changes or concerns.Team Collaboration : Work alongside passionate healthcare professionals, providing a supportive and compassionate environment for patients and their families.Personal Hygiene Assistance : Help with the patients personal hygiene needs, including bathing, dressing, grooming, and oral / mouth care.Light Housekeeping : Perform light housekeeping tasks related to the patient, such as making the patients bed and tidying the patients immediate environment.Adaptability : Be flexible and adaptable, ready to adjust to changing patient needs and care requirements.Minimum Requirements
Education / Certifications
Has successfully completed Nurse Aide training / competency evaluation program approved by the state and listed in good standing on the state nurse aide registry or state licensure program that meets requirementsMust have a CNA licenseReliable transportation with valid license & auto insurance.Experience
One year experience preferred, as a Hospice Aide or Nursing Assistant in a hospital, nursing home, home health agency, hospice or long-term care facility.Knowledge Skills & Abilities
