Position Summary :

We are seeking an experienced and compassionate Licensed Clinical Social Worker (LCSW) for an evening shift position at a reputable healthcare facility in Hyannis Massachusetts . The Social Worker will provide clinical assessments counseling discharge planning and coordination of care for patients requiring behavioral health and psychosocial support.

This role requires strong communication skills clinical judgment and the ability to work collaboratively within a multidisciplinary team to ensure quality patient care and continuity of services.

Key Responsibilities :

Conduct comprehensive clinical assessments for individuals with acute care or behavioral health needs in compliance with state and facility standards.

Develop and communicate Initial Treatment Plans to multidisciplinary teams based on assessment findings.
