Description

The Occupational Therapist / Physical Therapist, Certified Hand Therapist, is responsible for providing direct patient care, evaluations, consultations, and treatments to assist patients in reaching maximum physical performance for the upper quarter of the human body. Utilizing specialized skills in assessment and treatment, the Certified Hand Therapist promotes the goals of prevention of dysfunction, restoration of function, and / or reversal of the progression of pathology to enhance participation in life situations for individuals with upper quarter disease or injury. The Certified Hand Therapist may work in more than one outpatient setting in Allen County.

Education

Degree of Occupational Therapy or Physical Therapy from an accredited institution

Experience

If the applicant does not have a CHT certification, experience in Hand Rehabilitation / splinting is preferred.

Certifications and Licenses

Current certification as a Certified Hand Therapist, preferred

Applicant willing to obtain CHT license after hire, required

Summary : PT

Provides professional patient care and practices physical therapy by evaluating, planning, implementing, and documenting treatment. Evaluates and treats patients in accordance with the order of the physician and provides treatment through the use of a variety of therapeutic procedures and assistive devices intended to maximize functional potential in all areas of the patient's life. Develops and implements an individualized treatment plan based on patient needs and containing achievable functional goals. Revises treatment plans, as needed, throughout the treatment process. Instructs and collaborates with Physical Therapist Assistants (PTA) regarding treatment procedures to be utilized and provides the appropriate supervision of patients being treated by the PTA. Establishes and educates patients, families and other caregivers in exercises and therapeutic home programs to be continued following discharge. Completes the appropriate documentation and billing in a timely manner. All care is provided in accordance with the philosophy and essential functions of the hospital, the rehabilitation department, the Indiana State Practice Act, the American Physical Therapy Association, and The Joint Commission.

Education : PT

Must be a graduate of an accredited school of physical therapy, with a physical therapy degree as follows : Minimum of a Bachelor’s degree, if graduated prior 1 / 1 / 2016; Doctorate of Physical Therapy required, if graduated after 1 / 1 / 2016; International Candidates must be a graduate of an accredited school and possess a minimum of a Bachelor’s degree in Physical Therapy. Doctorate of Physical Therapy degree preferred.

Licensure / Certification : PT

Must hold or be eligible to receive a Physical Therapist license in the State where services will be provided. State licensure must be obtained within the first 90 days of employment. Must be certified in CPR or must become certified within the first 90 days of employment. Must maintain all certifications and license according to the expiration date. American Physical Therapy Association membership preferred. If assigned to drive a company vehicle, a valid driver's license is required. If assigned to drive a personal vehicle, proof of insurance is required in addition to a valid driver's license. Certified Hand Therapist (CHT) strongly preferred, with emphasis on hand, elbow, and shoulder rehabilitation.

Other Qualifications : PT

Must be able to lift 50 pounds, combined lift 100 pounds and push 300 pounds. Must have good verbal and written communication skills. Must be willing to work at other locations as assigned.

Assesses plans and carries out programs of purposeful activity designed to assist in the rehabilitation of medically necessary patients as prescribed by the physician. Bases selection of activities from physician's prescription, history, evaluation findings, experience and interests of the patient; plans and directs special diversion or educational events for patients. Instructs and collaborates with occupational therapy assistants regarding treatment procedures to be utilized. Supervises Certified Occupational Therapy Assistants / Occupational Therapy Assistants (COTA / OTA) in carrying out an OT program or selected parts of it. Completes all documentation and billing in a timely and acceptable manner. All care is provided in accordance with the philosophy and essential functions of the rehabilitation department, The Joint Commission, State Practice Acts, and the American Occupational Therapy Association.

Must be a graduate of an accredited school of Occupational Therapy, with an Occupational Therapy degree as follows : Minimum of a Bachelor’s degree if graduated prior 1 / 1 / 2002; Master's degree required if graduated after 1 / 1 / 2002; International Candidates must be a graduate of an accredited school and possess a minimum of a Bachelor’s degree in Occupational Therapy. A Masters degree is preferred.
