Key Responsibilities :
- Conduct thorough medical assessments, including history taking, physical examinations, and functional evaluations for geriatric patients.
- Diagnose and manage acute and chronic medical conditions common in older adults, such as hypertension, diabetes, heart disease, arthritis, and cognitive disorders.
- Develop and implement individualized care plans that promote patient safety, independence, and overall well-being.
- Coordinate and communicate with interdisciplinary teams, including primary care providers, specialists, nurses, therapists, and social workers.
- Prescribe medications and monitor for potential interactions, side effects, and adherence, with attention to polypharmacy concerns.
- Order, interpret, and integrate diagnostic tests such as labs, EKGs, imaging, and cognitive assessments into clinical decision-making.
- Provide patient and family education on disease management, preventive care, nutrition, and lifestyle modifications.
- Conduct wellness visits and participate in fall risk assessments, medication reconciliation, and advance care planning discussions.
- Document all clinical encounters, assessments, and care plans accurately and timely in the electronic medical record (EMR).
- Participate in quality improvement initiatives and continuing education to remain current on geriatric care best practices.
Qualifications :
Master’s degree from an accredited Physician Assistant program.Current NCCPA certification and active state license as a Physician Assistant.Minimum 1–2 years of clinical experience preferred, ideally in internal medicine, family medicine, or geriatrics.Strong understanding of age-related physiological changes, chronic disease management, and end-of-life care.Excellent communication, empathy, and teamwork skills, with sensitivity to the physical and emotional needs of older adults.Proficiency in EMR systems and documentation standards.Company Description
