Key Responsibilities :

• Conduct thorough medical assessments, including history taking, physical examinations, and functional evaluations for geriatric patients.
• Diagnose and manage acute and chronic medical conditions common in older adults, such as hypertension, diabetes, heart disease, arthritis, and cognitive disorders.
• Develop and implement individualized care plans that promote patient safety, independence, and overall well-being.
• Coordinate and communicate with interdisciplinary teams, including primary care providers, specialists, nurses, therapists, and social workers.
• Prescribe medications and monitor for potential interactions, side effects, and adherence, with attention to polypharmacy concerns.
• Order, interpret, and integrate diagnostic tests such as labs, EKGs, imaging, and cognitive assessments into clinical decision-making.
• Provide patient and family education on disease management, preventive care, nutrition, and lifestyle modifications.
• Conduct wellness visits and participate in fall risk assessments, medication reconciliation, and advance care planning discussions.
• Document all clinical encounters, assessments, and care plans accurately and timely in the electronic medical record (EMR).
• Participate in quality improvement initiatives and continuing education to remain current on geriatric care best practices.
