Job descriptionMember of interdisciplinary team providing and managing patient care using nursing process. Assess, diagnose, plan, implement, and evaluate care, catching complications early. Rapid assessment and interpretation of complex patient data. Make evidence-based decisions under pressure managing acute and chronic conditions. Assess patient's status through interview, observation, and physical assessment. Develop individualized plan of care collaboratively with patient / family and healthcare team. Use appropriate interventions to achieve expected patient outcomes. Evaluate and document patient's progress toward goals throughout the shift. Modify plan of care based on evaluation and patient condition changes. Complete required documentation including patient chart and event reports. Obtain adequate lab specimens for testing and perform bedside lab testing with quality control. Adapt teaching strategies based on patient's response, readiness, and comprehension. Consider cultural, age-specific, gender, and developmental factors in patient teaching. Ensure patient safety by following policies and National Patient Safety Goals. Utilize SVI patient education system as a resource for patients and families. Ensure care is consistent with standards, evidence-based practices, patient safety goals, core measures, and quality initiatives. Participate in Performance Improvement initiatives. Promptly follow up on patient / family requests and anticipate needs. Serve on unit-based councils.
