Multi-State Licensed Nurse Practitioner

A company is looking for a PRN Remote Nurse Practitioner.

Key Responsibilities

Oversee monitoring of patient vitals, symptoms, and labs in collaboration with RNs and MDs

Manage treatment plans and medications, focusing on guideline-directed medical therapy for chronic conditions

Lead virtual check-ins with patients and respond to escalations identified via remote patient monitoring

Required Qualifications, Training, and Education

5+ years of experience treating chronic diseases (T2D, Hypertension, CHF) in a medical setting

Active multi-state compact RN license and ANCC or AANP certification

Master's Degree as a Nurse Practitioner

Experience managing CHF with guideline-directed medical therapy

Stable home office setup with strong Wi-Fi for remote work
