NO CALL Office Hours are 8 : 00am - 5 : 00pm Requirements : ACLS, BLS, Board Certified or Eligible, Anesthesiologist or PM&R Residency Training, Interventional Pain Medicine Fellowship Required (ACGME) Advanced Procedures : (e.g., at least several of SCS, MILD, Kyphoplasty, Vertiflex, Minuteman, Viadisc, Discogram, Posterior Lumbar Fusion, SI Fixation) Payor Mix : Commercial, Medicare, Tricare, W.Comp (No Medicaid) Patient Volume : Varies between Surgery Center and Clinic Compensation : Initial Term : 2 years Guaranteed Base Salary : $Competitive Production Bonus : 40% of Collections over $1M Practice Ownership Opportunity : Partnership-track role with co-investment eligibility Benefits : Medical, Vision, Dental, ST / LT Disability, Life Insurance, 401K Match CME Allowance : $5K Business Development Allowance : $5K Malpractice Insurance : Covered by Practice $1M / Occurence, $3M Aggregate License, Assoc.
