License : Vermont Occupational Therapist licensure

Certifications : Home Health experience preferred, EMR experience preferred

Description :

Occupational Therapist needed to provide one-on-one therapy care for patients in their home setting.

Collaborate with an interdisciplinary team and colleagues to ensure a person's needs are met.

Provide occupational therapy care to a broad cross-section of clients.

Offer education and support to individuals to help them set and reach their goals of care.

Support clients' desire to maintain their independence and to live in their own home.

Must have home health experience.

Must have experience with EMR systems.

No requested time off is preferred.

Holiday coverage is required.
