Job Highlights

As a Hospice Social Worker, you will play a vital role in our interdisciplinary team by :

  • Participating in the coordination of patient care.
  • Contributing to comprehensive assessments and care plans.
  • Collaborating closely with physicians and the interdisciplinary team to execute care plans.
  • Ensuring compliance with policies, procedures, state / federal regulations, and accreditation standards.

    • Qualifications & Skills

  • Education :

    • Master of Social Work (MSW) from a program accredited by the Council on Social Work Education

  • Experience :

    • Two years of social work experience in a healthcare setting

  • Other Requirements :

    • Excellent communication skills.

  • Current CPR certification.
  • Valid driver's license and proof of automobile insurance.
