Travel Medical Social Worker - $2,173 per week in Seattle, WA

License : WA LICSW, WA LSWAIC, or WA LASW required upon submission; temporary licensure accepted. Certifications : BLS, MSW required. Must-Have : Experience in social work with geriatric clients. Description : This is a temp to perm position. Candidates who are interested in accepting a permanent role after a 13-week contract are encouraged to apply. This is a floating social work position supporting two Adult Day Care Programs. The social worker will float between two designated locations. As a member of the interdisciplinary team, this role provides social work services to participants. Services are delivered in a manner consistent with the mission and core values of the organization. The position involves a majority of time engaged in direct service activities. Direct service activities include one-on-one and group counseling. The social worker will interact with team members and provide ongoing assistance to participants in addressing social issues. Based on comprehensive assessments, the social worker provides insight into various factors impacting care. These factors include social, emotional, cognitive, and other relevant elements. The social worker will create individualized integrated plans of care for participants. Accessing community and financial resources for participants is a key responsibility. Ensuring continuity of care across the continuum is essential. The social worker conducts assessments and interventions in various settings, including participants' homes as needed. Some home visits will be required, but the amount of fieldwork is moderate to light. Home visits will not be scheduled in connection with personal commutes. The social worker will have autonomy in scheduling home visits. Mileage and hours spent driving between locations will be reimbursed and counted as paid time. All travel is expected to occur during the workday. Providers will be responsible for utilizing equipment such as laptops and cell phones to perform their duties.
