Full job description Position Summary

The Community Liaison is responsible for building strong relationships withreferral sources, healthcare providers, and community organizations to increaseawareness and utilization of Applied Home Health Network’s services.This role combines outreach, education, and client engagement to ensure that families,physicians, hospitals, and other stakeholders understand the full range of AHHN’sskilled nursing, pediatric, and non-medical services.

This role plays a pivotal part to supporting our mission of better care with compassion.

Key Responsibilities : Develop and maintain strong referral relationships with physicians, hospital discharge planners, nursing facilities, managed care organizations, social workers, and community agencies.

Promote Applied Home Health Network’s services through professional presentations, networking events, health fairs, and community programs.

Conduct regular outreach visits to referral sources, ensuring timely follow-up and strong communication.

Collaborate with the Intake and Clinical teams to ensure smooth referral-to-admission transitions for new clients.

Track and report referral activity, conversion rates, and outreach effectiveness using CRM tools and internal reporting.

Provide education to families, caregivers, and providers about AHHN’s care options, insurance coverage (Medicare, Medicaid, private pay), and specialty programs.

Identify community needs and emerging healthcare trends to support business growth strategies.

Represent AHHN with professionalism and adherence to all federal, state, and agency compliance standards.

Qualifications : Associate or bachelor’s degree in healthcare, marketing, business, or related field (preferred).

Previous experience in healthcare outreach, community engagement, or referral development within home health, hospice, or senior care.

Knowledge of Medicare, Medicaid, and private insurance programs.

Excellent interpersonal, presentation, and relationship-building skills.

Strong organizational and time-management abilities; able to meet goals with minimal supervision.

Event Coordination at community events, outreach activities, and informational sessions to promote agency services and strengthen relationships with local partners.

Valid driver’s license, reliable transportation, and ability to travel within assigned territory.
