Salary: $70,000 – $80,000 (Based on Experience)

In this role, you will provide client-centered social work services, support families, coordinate resources, and ensure clients receive the care and support they need to maintain independence and quality of life.

What You’ll Do

• Conduct psychosocial assessments of clients and families
• Perform home visits to evaluate living environments
• Develop and implement individualized care plans
• Serve as the main point of contact for clients and families
• Coordinate community resources and support services
• Lead client and family meetings
• Collaborate with clinical and interdisciplinary teams
• Educate families on care plans and available resources
• Ensure accurate documentation and compliance

What We’re Looking For

• Active New York State LMSW or LCSW license
• Master’s Degree in Social Work (MSW)
• 1+ year of experience in healthcare or social services (home care preferred)
• Strong case management and care coordination skills
• Excellent communication and interpersonal skills
• Ability to manage caseloads in a fast-paced environment
• Compassionate, patient-focused approach to care
