First Connect Health is seeking a Home Health LPN / LVN for a travel job in Peoria, Illinois.

OSF ITEMS OF NOTE :

Local Rate : Applies to clinicians with permanent addresses within 50 miles (-$25 to regular and OT bill rates)

OSF Call Off Policy : Equivalent of 12 hours every 13 weeks with no penalty or need to pay for the shift

Non-billable Orientation Hours : 12 hours, guaranteed hours will not apply during orientation

Department : Home Health | LPN

Required Certifications : BLS. Valid driver's license & insurance.

Common diagnosis / Types of patients / patient demographics : Disease management, Wound Care, Medications.

Experience or 'Must have' skills : EPIC Remote Client EMR experience.

Ability to work independently to manage patient load, communicate with team, and provide great patient care skills.

Home health and wound vac experience preferred.
